Healthcare Staffing Professionals has an immediate need for a Clinical Social Worker. The Clinical Social Worker provides professional social work services to patients and families presenting difficult and complex medical and related psycho-social problems; or, in addition to providing direct patient services, has responsibility for the social work activities of a small social work unit in a community ambulatory care setting.

PROGRAM OVERVIEW

Enhanced Care Management (ECM) is a Medi-Cal managed care benefit that addresses clinical and non-clinical needs of high-need individuals through the coordination of services and comprehensive care management.Childrens Medical Services (CMS) administers the California Children Services, the Medical Therapy Program, the Child Welfare Public Health Nursing Program, and the CMS Edelman Childrens Court Pediatric Program.ECM-CMS program will serve this particularly vulnerable ECM population of focus (POFs): Children/Youth Enrolled in California Childrens Services (CCS) with Additional Needs Beyond the CCS Condition. DPHs ECM-CMS team has extensive experience and expertise in working with this population for intensive care management and care coordination. It is anticipated that some CCS clients are also in the Child Welfare System. These clients, when encountered will be included in the initial launch. After the initial successful program launch, ECM-CMS will consider growth to fully serve both POFs. Intensive care management will be provided based on comprehensive care plans that aim at addressing the need for overall care management for the multiple systems with which the child/youth and family interact.

  • The positions will start immediately upon acceptance and employment clearance and are initially funded through 12/31/26, with the good chance of extension.

Pay Rate: $40.15/hr working 40 hours a week

Location: Hybrid 3 Days a week in office and 2 days telework

Job Duties

Clinical Social Workers (CSWs) supervise teams of lead care managers (medical case workers- MCW). Each CSW can supervise up to seven MCWs: a supervisor-to-subordinate ratio based on DPH/CMS experience with social work/casework ratios in other areas of service delivery. In addition, the CSW will supervise two clerical staff (one dedicated to case management support and one to provide data reporting and entry for support for the team). As team leaders they will coordinate care management functions within the ECM-CMS program by providing professional diagnostic and goal setting functions for the team member.

  • Assigning cases to their local care lead care managers (LCMs)
  • Advising and counseling their LCMs
  • Reviewing all care plans and updates with their LCMs
  • Identifying and accessing community, support, welfare and behavioral programs for their LCMs clients
  • Monitoring overall and individual client outcomes and barriers;
  • Coordinating clients/families access to behavioral health services (e.g., mental health/substance use)
  • Teaching MCWs how to identify potential behavioral health issues complicating family dynamics
  • Partnering with behavioral health programs on behalf of DPH/CMS ECM; and Setting behavioral health standards for the care management teams.

Requirements

  • Master's Degree in Social work from a graduate school accredited by the Council on Social Work Education including supervised clinical social work fieldwork experience

- OR - A Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education and one year of experience in medical, psychiatric, family, or child welfare social work involving problems of medical, mental, or emotional adjustment.

LICENSE: A valid, current license as a Licensed Clinical Social Worker issued by the California Board of Behavioral Sciences or registration in good standing as an Associate Clinical Social Worker with the California Board of Behavioral Sciences within six (6) months of employment. Associate Clinical Social Workers must remain in good standing with the California Board of Behavioral Sciences for the duration of the waivers permitted by law, not to exceed six years.

  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-relate
